Certification of Financial Responsibility Form

Non-US applicants to the UF Graduate School must fill out and submit this form as part of the application process, in order to qualify for a student visa.

Degree Application Form

Log in to the UF Office of the Registar's ISIS website to fill out a degree application online before the deadline of your graduating semester. For deadlines, click here: Critical Dates.

Degree Certification Status Verification Form

If you need a letter verifying your current degree candidate status, fill out this form and submit it to UF Graduate School Student Records.

Fellowship/Assistantship Application Form

Fill out this form and submit it to the chair of your department if you are applying for a fellowship or assistantship.

Late Degree Application Form

If you miss the degree application deadline for the semester in which you planned on graduating, fill out this form, get UF Graduate School approval for it, and then submit it to the UF Office of the Registrar before the semester midpoint published in the Critical Dates section of the UF Graduate Catalog. If you miss the semester midpoint deadline for this form, you must graduate the following semester by submitting a degree application form for that semester.

Recommendation Letter Form

Use this form if you are applying to the UF Graduate School. Recommendation letters are to be mailed directly to the department to whose master's degree or doctoral degree program you are applying: College and Department Links.

Traveling Scholar Form

Fill out this form according to its instructions if you are applying to participate in the UF Traveling Scholar Program as a UF graduate student attending an institution of higher learning elsewhere, either within the Florida State University System or outside of the Florida State University System — but within the United States.
